First seven days home
Use this list to organise information and family actions after discharge. It does not replace the discharge instructions or tell anyone what care to provide.
Before leaving hospital
- Collect the supplied documentsDischarge letter, medicine list, appointment details and contact instructions.
- Ask where questions should goWrite down the service name, number and available hours exactly as supplied.
- Name the first family contactOne person gathers updates and records who owns each next action.
First day home
- Make one current handoverRecord what changed, what was done and what remains open.
- Keep supplied wording with its sourceDo not rewrite uncertain medicine or appointment details from memory.
- Assign every open family actionName one owner and a due time where useful.
Days 2–3
- Review open arrangementsTransport, supplies, follow-up calls and documents to take.
- Record factual changesInclude who reported the change and when.
- Check accessRemove people from chats or shared files when they no longer need the details.
Days 4–7
- Prepare for the next appointmentGather questions, source documents and incomplete family actions.
- Correct the record visiblyKeep the original wording and mark the later factual correction.
- Agree the next handover timeDecide when the family will review the current information together.
